SUMMARY

The Home Care Registered Nurse provided skilled nursing care in! cluding skilled assessment and instruction in the home under a plan of care/treatment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Include the following; however other duties may be assigned:
Evaluates patient care needs on admission and on an on-going basis:

Demonstrates knowledge of and adherence to Agency policies and applicable regulations and standards;

Demonstrates knowledge of an application of principles of the nursing process;

Evaluates nursing plan on an ongoing basis, communicates with the physician and makes referrals to other members of the health care team as necessary.

Plans, coordinates and implements skilled care according to established nursing standards and makes revisions as required, under the physician’s plan of treatment:

Performs duties consistent with the California Nursing Practice Act;

Utilizes the principles of the nursing care process;

Coordinates care of ! patient with other care providers through communication with C! ase Manager, other team members and documentation;

Uses community resources as appropriate;

Reviews and signs documentation of Licensed Vocational Nurse when applicable;

Provides clear, concise written instructions for home health aide, as appropriate.

Notifies physician and other appropriate Agency personnel of changes in the patient’s condition:

Communicates changes in the patient’s condition to the physician in a timely manner;

Reports changes in the patient’s condition to the Director of Patient Care Services and/or other Agency personnel as appropriate.

Instructs and supervises nursing personnel regarding the needs of the home care patient:

Provides supervision consistent with California Nursing Practice Act;

Provides clear, concise written instructions for the home health aide, as appropriate;

Supervises the home health aide according to applicable regulations, standards an! d Agency policy;

Communicates with other members of the team regarding patient care needs.

Instructs and counsels patient and family regarding nursing needs and community resources:

Utilizes appropriate communication skills to instruct and counsel patient and family regarding care at home;

Demonstrates knowledge regarding community resources and ability to communicate knowledge to patient and family.

Documents patient assessment and on-going care concisely according to Agency policy and nursing standards;

Documents care clearly using accepted standards of documentation;

Prepares admission and assessment forms, plan of treatment form, clinical and progress notes, and other appropriate forms according to applicable regulations, standards and Agency policy.

Participates in the quality improvement program

Prepares materials accurately for quality improvement, as requested;

Participates in ! process improvement.

Participates in the health and safety ! program:
Consistently practices safe work habits;

Consistently reports unsafe working conditions.

Demonstrates a commitment to professional growth:
Attends appropriate in-services;

Demonstrates self-directed learning to meet professional development and licensure requirements;

Performs other duties as assigned by supervisor:
Demonstrates willingness to accept other assignments;

Completes assignments effectively.

Required Experience

EDUCATION AND EXPERIENCE, SKILLS, CERTIFICATES, LICE! NSES, AND/OR REGISTRATIONS:

Education: Bachelor’s Deg! ree in Nursing required; Graduate of an NLN accredited school of nursing preferably.

Experience and Skills: One year prior professional nursing experience as a Registered Nurse.

Certificates/Licenses/and/or Registrations, etc..: Current Registered Nurse licensure; CPR Certificate

Other Requirements: A tuberculin clearance at the time of employment and every year thereafter; A health examination within six (6) months prior to employment or within fifteen (15) days after the date of employment; A reliable automobile; Driver’s License; Evidence of automobile insurance; Current CPR certificate.

WORK ENVIRONMENT:
General Office or client's home

PHYSICAL AND OTHER DEMANDS: Employee is required to hear, speak, sit, stand, and walk daily. Employee may lift up to 50 pounds.
