Field RN HH/Casemanager

Requires driving 60 mile radius from agency. Weekend on call rotation. Must have reliable transportation. Census 191, 3FTE: 5FTE, S&W% 46.15%: 50.25 Responsible for the delivery of patient care through the nursing ! process of assessment, diagnosis, planning, implementation, and evaluation. Responsible for directing, coordinating all nursing care based on established clinical nursing practices. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Requires driving 60 mile radius from agency. Weekend on call rotation. Must have reliable transportation.
Qualifications:
EDUCATION/LICENSURE/CERTIFICATION/SKILLS REQUIREMENTS PREFERRED:
Licensed Registered Nurse in the state of practice. BSN preferred.
EXPERIENCE PREFERRED:
Minimum of one year clinical experience in the hospital setting, home health care nursing experience also preferred. Six months supervisory experience preferred. Must possess knowledge of Medicare, Medicaid, and CHAP regulations. Strong communication skills and leadership skills required. Attention to detail and good organizationa! l skill needed.
Licenses:
RN license in st! ate of practice, current driver's license, current auto insurance, current CPR card.
