Position provides continuity between discharge from other acute and post-acute care settings and admission for Hospice Services.

Minimu! m Position Qualifications:
Education:
Graduate of an Accredited School of Nursing.

Experience:
At least two (2) years experience as a Nurse.

Training:

• Eighteen (18) hours of Hospice training.
• Six (6) hours of Bereavement training.

License/Certification:

• Current license as a Registered Nurse
• CPR certification
• Driver's license
• Proof of current auto liability insurance.
