POSITION SUMMARYThis position requires travel within a defined geographic area on a daily basis to physician practices and offices. Applic! ant must be a resident of Texas and reside in the Sugar Land/Houston, McAllen, El Paso, or Lubbock areas.The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individuals benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes. Requires an RN with unrestricted active license

Nurses in this position will be responsible for working to develop clinical relationships with providers in the community to make them aware of Aetna Medicare programs and resources that may benefit both them and their Aetna Medicare members. The nurse will outreach to primary care physicians and to physicians/offices who work with patients with chronic illness (e.g., endocrinologists, cardiologists, pulmonologists, nephrologists, gastroenterologists, etc.).

The overall goal is to identify opportunities to enhance clinical engagement with! the physician group's members in order to improve quality, me! mber experience, and efficiency of care for Medicare members in the local geography.

Requires an RN with unrestricted active license. Managed care industry experience desirable. Medicare and/or Medicare Care Management experience desirable. Sales experience to include program presentations demonstrating effective communication skills, ability to problem solve, meet productivity criteria and experience with territory management desirable
EDUCATION
The highest level of education desired for candidates in this position is a Associate's degree or equivalent experience
