Description: Virginia's Hospice is currently interviewing Hospice CNA (Certified Nursing Aide) right now, this job will be placed in Texas. For detail informations about this job opportunity kindly see the descriptions. Virginia°s Hospice is hiring CNA°s for our case manager position immediately. We are a family-owned and operated Hospice that is rapidly growing in the Houston/North Houston area. ! In addition to the hospice, we also own and operate a 16-bed assisted living facility. Our main office is located in Willis, TX (a couple miles north of Conroe), with administrative offices in Fairfield and Houston (Galleria area).

This particular position will be serving the greater Conroe/Woodlands/North Houston area. Roles/Responsibilities: -90% of the time, your usual schedule will be from 8AM-5PM, unless there is a facility or patient request visits outside of normal hours -Visit patients as dictated by the patient care plan (we keep each CNA°s case load at 10 or less patients). -Attend family/patient funeral services when available -Chart all visits, in our electroni! c medical record system -Coordinate with the RN case manager f! or patient care Hospice, home health, or private duty experience is required.
