The Staff Nurse II/Graduate Nurse performs complex nursing work at the proficient or expert level in a state mental health hospital. Works with limited supervision from the Charge Nurse or Nurse Manager and has considerable latitude for the use of initiative and independent judgment. Acts as Charge Nurse, Nurse Supervisor, or Nurse Manager in his/her absence when necessary. Provides clinical supervision to Nurse I, Licensed Vocational Nurses and Mental Health Workers. Organizes, establishes, and maintains unit routines and the therapeutic milieu. Coaches and teaches subordinate staff in the provis! ion of population-specific nursing care and treatment. Supervi! ses the administration of or administers medications in compliance with facility policies and procedures. Conducts extensive assessments of the patientsâ nursing care needs including initial and ongoing assessments. Develops, implements, or supervises the implementation of population-specific nursing interventions based on acceptable nursing standard of care and relevant evidence. Acts as a nursing team leader in collecting patient data and contributing appropriate information to the formulation of the patientâs interdisciplinary treatment plan. Must understand that duties, location of work, shift/pattern are subject to change as determined by the administration to meet the needs of the hospital. Must understand that duties may include exposure to blood or other potentially infectious material.Employee actively participates and/or serves in a supporting role to meet the agencyâs obligations for disaster response and/or recovery or Continuity of Operations (C! OOP) activation.
Essential Job Functions:
1. Nursing interventions in accordance with facility policy and procedures. (5%)
Develops an implements training for subordinate staff regarding Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
2. Performs extensive initial and ongoing assessment of the patientâs nursing care needs to include medical, psychological, and social needs. (20%)
3. Develops nursing diagnoses or problem lists based on the assessment of the data and formulates the initial plan of care (10%).
4. Develops and implements population specific nursing interventions based on accepted nursing standards of care and through review of current evidence (10%).
5. Collaborates with other health care professionals by contributing nursing assessment date, diagnoses/problem lists, and interventions to the development of the multidisciplinary treatment pl! an. (10%)
6. Evaluates the outcomes of nursing interventions and do! cuments the same in the medical record (5%)
7. Provides comprehensive patient/family education regarding illness process, medications, coping skills, etc. to patients and families. (5%)
8. Establishes, monitors, and maintains the therapeutic milieu through clinical supervision and training of other Registered Nurses, Licensed Vocational Nurses, and Mental Health Workers. (5%)
9. Administers or supervises administration of medications and other principles of population-specific nursing care, unit routines, facility policy and procedures, management directives, and milieu management, etc. (5%)
10. Acts as team leader in emergency situations (i.e. medical or psychiatric emergencies) including initiating and monitoring the appropriate use of resuscitative equipment or restraint/seclusion. (5%)
11. As assigned, acts as charge nurse, nurse supervisor, nurse manager in his/her absence. (5%)
12. Demonstrates a commitment to a culture of safety that promotes ! respect, dignity, and a reduction in risk to both patients and staff through support of the hospitalâs safety and risk management plans, infection control programs, and restraints and seclusion elimination strategies (5%).
13. Responds proactively to avert crisis, but effectively in all psychiatric emergency situations to avoid need for physical intervention (5%).
14. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agencyâs obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location (5%).
Monitors prevention and Management of Aggressive Behavior (PMAB) 1 â" 6 and, in the absence of other staff, implements PMAB 1 â" 3.2
Knowledge Skills Abilities:
Extensive knowledge of nursing procedures and practices. Extensive knowledge of the Texas! Nursing Practice Act, Rules and Regulation of the Board of Nurse Exami! ners for the State of Texas. Ability to provide population-specific, evidence-based nursing care at the proficient or expert level to an assigned patient population. Ability to perform an extensive assessment of the patients nursing care needs to include medical, psychological, and social needs. Ability to formulate population-specific nursing diagnoses for the assigned patient population. Knowledge of leadership or management principles as they apply to the unit or hospital setting. Knowledge of teaching principles in order to provide both patient and staff education. Ability to communicate effectively with others in one-to-one or group interactions. Comprehensive knowledge of safe and appropriate use of restraint or seclusion as an intervention in the psychiatric setting. Ability to develop and implement evidence-based nursing interventions appropriate to the assigned population. Ability to provide leadership in the management of both medical and psychiatric emergencies. ! Ability to obtain certification for basic Cardio-Pulmonary Resuscitation (CPR) and ability to perform and/or supervise the implementation of CPR in the clinical setting. Ability to complete PMAB 1 â" 32 prior to working on a patient care unit.
Registration or Licensure Requirements:
Licensed to practice as a professional nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact or Graduate Nurse with a valid Texas permit.
Initial Selection Criteria:
Licensed to practice as a registered nurse in the State of Texas or state that recognizes reciprocity through the Nurse Licensure Compact, or possess a temporary permit to practice nursing in the State of Texas. Graduate nurses welcome.
