PURPOSE: Leads the clinical outreach team that is responsible for meeting initiatives set forth by health plan contracts. These initiatives will vary from contract to contract. Works collaboratively with the Outreach Unit Business Specialist, Medical Director and Program Director. Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization.

ESSENTIAL FUNCTIONS:
Reviews and validates list of patients for contact.

Contacts patients via telephone, using scripts.Reviews reports and prioritize patients for contact by clinical vs. administrative.

Contacts targeted patients, develops plans of action, ensures physician’s care plan is followed, monitors status of these plans.

Provides patients education and materials as needed.

Develops “registry” and documentation process.

Interacts/communicates with ARC physicians and nurses as well as non-ARC specialists and staff.

Coordinates patient interactions with Tenzing and MediView (vendors).

Determines if patient qualifies for copay/medication support.

Visits patients at home/work if needed â€" this would include assessment and perhaps some “treatment” under medical director’s orders.

OTHER DUTIES AND RESPONSIBILITIES:
● Performs other duties as assigned.

MINIMUM QUALIFICATIONS:
Education: Graduate of accredited nursing school.

Experience: 3-5 years working in direct patient care in clinical environment. Experience in case management or administrative experience helpful.

Knowledge, Skills, and Abilities:
Excellent verbal and written communication skills.

Strong organizational skills.

Ability to work in a collaborative, team oriented environment.

Exceptional level of critical thinking, decision-making, and problem-solving abilities.

Ability to influence others towards a shared goal (teamwork).

Ability to build rapport and trust with patients.

Bilingual in English and Spanish preferred.

Certificate/License: Licensed Registered Nurse in the State of Texas. Current AHA approved Basic Life Support (BLS) Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card.
