We have a new opportunity for a Case
Manager for a respected Skilled Nursing Facility in Dallas, TX. As a member of
the Case Management team, successful candidat! e will provide expertise, guidance, advocacy,
education, and direction to effectively assist in appropriate admissions and clinical case
management.

We are searching for a candidate with a strong clinical background and case management
experience within a Long Term Care environment. Registered Nurse candidates strongly
preferred.

Responsibilities include:
Performing admissions data, MDS, and clinical documentation review

Developing action plans to improve the Admissions process as needed

Reviewing Managed Care contracts for coverage and eligibility requirements

Maintaining current understanding of state and federal requirements

Assisting facility staff in determining appropriate coverage, and completing
documentation

Ensuring accurate assessment, timely transmissions, maximum utilization of resources, and
program integrity

Monitoring changes to Medicare / Medicaid prog! rams and adapting case management processes
appropriately !
