- San Angelo, TX: We need 2 providers for our LTC facilities visits and house calls.
The nursing home/skilled nursing facility based advanced provider works in conjunction with physicians in skilled nursing facilities to ensure patients are appropriately managed in the sub-acute an/or long-term care setting. The practitioner will provide direct (hands-on) care in nursing homes/skilled facilities with a goal to improve quality and prevent unnecessary re-admissions to the acute care setting.
Essential functions include, but are not limited to the following:
? Responsible for the treatment of members in the nursing home and skilled nursing facility settings. Performs comprehensive SNF ass! essment to include psychosocial factors (when applicable) iden! tifying the anticipated short-term and long-term needs of the member.
? Identifies and refers members into appropriate disease and case management programs. Identifies the need for hospice care as appropriate in the “terminal period†of chronic diseases.
? Refers appropriate members to case managers for hospice program election or to be designated as a “Potential Hospice†candidate and monitored for decline.
This provider collaborates with the physicians for all patient related responsibilities and deals directly with skilled nursing facility staff members to coordinate patient care requirements.

Qualifications and Requirements
The successful Nurse Practitioner or Physician Assistant candidate will have a certificate of completion f! rom an approved program and be eligible for or certified by their respective Texas board organizations. The candidate must be certified to work with the adult population age group. Must have current BLS. Previous experience in a skilled nursing facility or with geriatric patients is preferred.
The candidate should have excellent people skills, demonstrated concern for the health and well-being of others, superior written and oral communication skills, computer literacy including electronic medical record documentation and the ability to work both as a team member and independently.
