The Case Manager provides assistance to patients and families as related to illness, disease, and hospitalization in all areas of service! as a member of the multidisciplinary team, and in liaison with community resources.

Minimum Qualifications:

• Education: Graduate of an accredited school of professional nursing.
• Experience: 2-3 Years of Medical/Surgical Nursing practice in hospital setting or Case Management, discharge planning, utilization review or related experiences.
• Licensure: Current licensure as an RN, and Certificate in Case Management required.

Job Summary/Purpose: Collaborates with patients, families and healthcare team members by assessing the client’s individual needs across the continuum of care: planning implementing and facilitating the services that he/she will require, evaluating the effectiveness of the services, and arranging services for discharge that will ease clients’ transition to the next level of care. The ideal candidate for this position is familiar with resources available in the Central Texas area and has expe! rience working with pediatric patients.

Compensati! on: Negotiable based on experience

Schedule: Monday â€" Friday, Standard Business Hours
