Job Description:
Responsible for implementing a plan of care as delegated by the Nurse Manger to assigned client(s). To perform nursing care and treatments, administer medications and monitor/observe client status. To respond appropriately to emergency situations. Operate under the direction of RN, MD and within the standards of the North Carolina Nursing Practice Act. There is significant risk for occupational exposure to bloodborne and airborne pathogens.

Minimum Qualifications:
LPN currently licensed in the state that they are working as a LPN. Current CPR certification. One year experience as a LPN with direct patient care, medical / surgical experience desired. Must have valid driver's license, reliable transportation, current inspection and insurance. Must be willing to travel as needed. Must be capable of maintaining regular attendance

Preferred Qualifications:
Able to set priorities and implement plan of care. Competent in technical skills, critical thinking client observation and communication with client/family, peers and supervisor. Must be able to maintain confidentiality regarding patient and company proprietary information. Must be able to relate professionally and positively to corporate staff and to work cooperatively with other employees at all levels. Must possess computer skills at a level necessary to accomplish the job

Essential Job Functions:
Accept care, review nursing assessment, plan of care and problem list, and acknowledge understanding of care prior to reporting to case. Perform all nursing care and interventions according to nursing plan of care and physician authorization. Administer and document medications as ordered. Work with client, family and Nurse Manger to accomplish goals of care plan. Maintain professional and positive working relationships with client, family and members of health care team. Monitor client condition and report changes to Nurse Manager.Participate in planning client care by providing information and suggestions to achieve client goals. Document all care and client's response to care. Report client change in condition and unusual occurrences to Nursing Supervisor, and MD if appropriate, and document. Maintain current and competent clinical skills including technical skills, safety and infection control, and emergency procedures. Seek new knowledge and experience and utilize them in home care. Participate in the CareFocus Employee Suggestion Program. Participate in inservices and attend meetings as needed. Work as a positive member of the CareFocus team. Represent CareFocus in the community and client home in a positive professional manner. Report to work as scheduled. Uphold and support CareFocus policies and procedures. Accurately record time worked. Report weekly availability, flexible in schedule to assist in providing consistent care to client. Contribute to team effort by accomplishing related results as needed.
