    This position is at Brooke Army Medical Center at Ft. Sam Houston, Texas.

    This job may on occasion entail evening and weekend duty. This job may entail occasional visits to patient's home or other treatment facilities as part of case management function.

    Brooke Army Medical Center (BAMC) at Fort Sam Houston, Texas, is proud to provide safe, quality care to our military service members, their families, veterans and civilian emergency patients as the most robust and productive healthcare organization within the Military Health System (MHS). Both inpatient and outpatient services are provided by approximately 8,500 staff members, including active duty military personnel from each of our uniformed services, Public Health personnel, federal civilians, contract employees, and volunteers. Fort Sam Houston is located in San Antonio, the second largest metropolitan area in Texas with a warm climate and close proximity to beaches. With its combination of art galleries, distinctive shops and fine dining experiences in a h istorically significant setting, the San Antonio River Walk, also known as the Paseo del Rio, comprises a network of walkways on the banks of the San Antonio River. Located underneath downtown San Antonio, this area contains shops, restaurants and other entertainment options.

    Responsibilities

    • Provide advanced practice clinical nursing, administrative and organizational skills in developing the plan of care and managing the continuity of care for the beneficiary populations.
    • Recognize problems with health care access, explore options to care when available, and strive to improve quality and delivery of care.
    • Identify and screen potential case management clients through health planning assessments and review of multiple databases.
    • Interview and counsels potential clients about case management, healthcare and treatment options, community reso urces, insurance benefits, and psychosocial concerns.
    • Provide technical oversight and input to nursing practice matters.

    • Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
    • Two year trial/probationary period may be required.
    • Selectees will be required to sign a statement (Condition of Employment) consenting to seasonal influenza vaccinations or must provide a recognized exemption.
    • This position is a Testing Designated Position subject to applicant testing and random drug testing. Selectees will be required to sign DA Form 5019 requiring participation in random drug urinalysis testing.
    • A pre-placement medical examination is required.
    • Immunization screening is required. Hepatitis B immunization is required for all positions with direct patient contact. Applicants may be required to show proof of other immunizations depending on the type of position.
    • Must be able to obtain and maintain current Basic Life Support (BLS) Training and certification sponsored or endorsed by the American Heart Association (AHA). Advanced certification (e.g., Advanced Life Support) does not supersede BLS completion.
    • Applicants must be able to fulfill credentialing requirements and obtain and maintain appropriate/relevant clinical privileges.
    • This position has specific initial background investigation and periodic re-verification requirements since its duties require int eraction with children and youth under the age of eighteen (18) years of age on a recurring basis.
    • Position is designated as Inclement Weather Essential. The incumbent must make every attempt to report for work on time and/or remain on duty during severe weather conditions.

    In order to qualify, you must meet the education and/or experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience.     Basic Requirement for Nurse (Clinical/Case Management):

    Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

    Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.

    Licensure: Applicants must have passed the National Council Licensure Examination and possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.

    In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

    Experience: At least one full year of professional nursing experience (equivalent to the 11). Professional Nursing Experience is defined as: One year of experience which includes developing patient care plans, conducting case management discussions with a variety of stakeholders, and evaluating appropriateness, quality, or cost effectiveness of care.

    Some federal jobs allow you to substitute your education for the required experience in order to qualify. For this job, you must meet the qualification requirement using experience alone-no substitution of education for experience is permitted.
